The Sacred Rebel respects the privacy and confidentiality of clients and takes the protection of personal and health information seriously. Information is handled with care and used where reasonably necessary to provide services and operate the business, or otherwise as permitted or required by law.

2. Information We May Collect

Depending on the service, information may include name and contact details; nutrition and dietary information; eating habits and food preferences; fitness and exercise history; activity levels; health and medical history; injuries or physical limitations; medications; information about previous or current practitioners; lifestyle information; goals; mindset and wellbeing information; questionnaires, assessments and relevant client notes. Some information may be sensitive or health information.

3. Why Information Is Collected

Information is collected to understand the client’s individual needs, circumstances and goals, determine an appropriate way of working together, support safe service delivery, and identify circumstances where another health professional may need to be consulted.

4. Pre-Service Questionnaire

Before certain services, clients may be asked to complete a questionnaire covering nutrition, fitness, health, lifestyle and mindset. It may be sent as a PDF by email and returned electronically or brought as a completed copy to the appointment.

5. Storage & Security

The Sacred Rebel primarily maintains client information in secure physical files with access restricted to the business. Where information is returned or communicated electronically, copies may also exist in email or other electronic communications. Reasonable steps are taken to protect personal and health information from misuse, interference, loss and unauthorised access, modification or disclosure.

6. Sharing Information

The Sacred Rebel does not ordinarily disclose client information to other people or organisations. Where it may be beneficial to communicate relevant information to a GP, physiotherapist or another practitioner, the client’s permission will ordinarily be obtained first. Information may nevertheless be used or disclosed where required or authorised by law or where another lawful exception applies.

7. Marketing

Client health information is not used for unrelated marketing purposes.

8. Testimonials, Photographs & Client Stories

A client’s testimonial, photograph, personal story or identifiable results will only be published or used for promotional purposes with the client’s permission.

9. Access & Correction

Clients may contact The Sacred Rebel to request access to personal information held about them or to request correction of information they believe is inaccurate. Requests will be handled in accordance with applicable privacy requirements.

10. Retention & Disposal

Personal and health information is retained for as long as reasonably necessary for the purposes for which it was collected and to meet applicable legal, professional, insurance or record-keeping requirements. When information is no longer required, reasonable steps will be taken to securely destroy or de-identify it where appropriate.

11. Overseas Disclosure

The Sacred Rebel does not ordinarily disclose client personal or health information to overseas recipients. If this practice changes materially, this Privacy Policy should be updated.
